Requirements for Applying for Assistance

In order for an application to be considered, we will have to have the following information submitted with the application.
You must allow 2 (two) weeks for application to be processed
The above information MUST be completed for Assistance mailed to:

If your situation has changed, please add an explanation and reason for the change.
NOTE: If you are not required to file a return, indicate beside "Monthly income"

APPLICATIONS WITHOUT THE REQUIRED VERIFICATION INFORMATION WILL NOT BE PROCESSED.
Nor will we contact you concerning any missing information. It is YOUR responsibility to submit the required information.

This can come from your Pastor, Social Worker, Employer, Government assistance agency or Counselor for example.
Pay stubs (yours, and spouses, along with anyone with income in the household), SSI or SSDI must have copy of reward letter, including that of dependant children, unemployment, child support, spouse support, food stamps, or Medicaid. Please list separately on additional paper. All household income must be included.
Please include copys of all current medical bills with Dates. (Care Credit Card not eligible for assistance)
Please include most current bill. Must be a disconnect notice. We understand the importance of Emergencies. However, we still need time to complete the application process. You MUST go through the COMPLETE application process it's required by the law.
Please submit Application, Agreement and Release, Privacy Policy, Copy of Diagnoise, and Proof of income.
